THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

OUR COMMITMENT TO YOUR PRIVACY

Dandelion Therapy Group (a DBA of Dandelion Behavioral Health, LLC) is dedicated to maintaining the privacy of your protected health information (PHI). We are required by law to maintain the confidentiality of health information that identifies you and to provide you with this notice of our legal duties and privacy practices.

HOW WE MAY USE AND DISCLOSE YOUR HEALTH INFORMATION

  • Treatment & Operations: We may use your PHI to provide therapy services, bill for those services, and conduct quality reviews of our practice.

  • Appointment Reminders & SMS: We may use and disclose your PHI to contact you as a reminder for appointments. If you have provided your consent, this includes contact via SMS/text messaging. Please be aware that standard SMS is not a secure or encrypted form of communication. By opting in, you acknowledge the inherent privacy risks of unencrypted text.

  • Substance Use Disorder (SUD) Records: If we maintain records protected by 42 CFR Part 2, these records receive heightened protection. They will not be used or disclosed in legal proceedings against you without your specific written consent or a court order.

  • Notice of Redisclosure: Health information disclosed to others (like an insurance company) may be subject to redisclosure by the recipient and may no longer be protected by the HIPAA Privacy Rule.

YOUR RIGHTS REGARDING YOUR PHI

  • Access & Amendment: You have the right to inspect, copy, or request a change to your clinical records.

  • Right to Restrict SMS: You have the right to request that we communicate with you in a specific way. You may opt out of SMS/text communications at any time by replying STOP to any message or by notifying us in writing.

  • Accounting of Disclosures: You may request a list of certain disclosures we have made of your PHI.

FILE A COMPLAINT

If you believe your privacy rights have been violated, you may file a complaint with our Practice or with the Secretary of the Department of Health and Human Services. You will not be penalized for filing a complaint.
